Critical Incident Reporting (CIR)

Community Living British Columbia (CLBC) defines “critical incidents” as serious or unusual events that involve an individual accessing CLBC-funded services that occur while service is being delivered (this includes staffed living and shared living at all times).

 

Unlike other incidents that occur during service delivery, service providers must report critical incidents to CLBC.

When & How to Report

When To Report to CLBC:

  • For urgent critical incidents that result in serious harm to an individual and/or require immediate action by CLBC or others:

    • Call CLBC immediately; and

    • Submit, by fax or email the critical incident report within 24 hours to the local CLBC analyst and office responsible for their contract.

  • ​For non-urgent critical incidents that do not result in serious harm or require immediate action by CLBC or others, you are required to submit the report within 5 working days to the local CLBC office responsible for their contract.

Please Note:  Our Quality Assurance Coordinator (QAC) will file formal CLBC Critical Incident Report to CLBC on your behalf. See below on the steps on how to report.

How to Report:

  1. Report Incident Immediately: Notify your Supervisor or Manager as soon as a critical incident occurs.

  2. Daily Charting: In your daily charting & journal entry log, under "Select Type of Entry being made below," choose: "Critical Incident Report (CLBC Form Still Required). Document the full interaction objectively, including a summary of the incident.

  3. Complete Internal CIR Form: You'll then be directed to complete an Internal CIR Form adapted from CLBC's version, to provide a more in-depth report of the incident. This form will be submitted to the Quality Assurance Coordinator (QAC).

  4. QAC Follow-Up: Our QAC may reach out for clarification & additional information.

  5. Submission to CLBC: The QAC will complete and submit the official CIR form to CLBC on your behalf.
